Committed to Caring our mission, a Home Health Aide is specifically trained, competent and performs the provisions of hands-on personal care, the performance of simple procedures as an extension of therapy or nursing services, assistance in ambulation exercises, assistance in administering medications ordinarily self administered (assistance is limited to getting water or fluids) to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). The Skilled professional staff whether direct employee of the agency or under arrangement are expected to contribute to all phases of the QAPI program and participates in with ongoing interdisciplinary assessments of patients. The Home Health Aide will participate in the development and revision of the plan of care in partnership with the patient, patient representative (if any) and caregiver(s).

Qualifications:

  • Meets one of the requirements of the Texas Administrative Code (state.tx.us); Chapter 558
  • Certified Nursing Assistant (CNA) in the state(s) in which practicing, preferred
  • CPR certified required
  • Ability to exercise initiative and independent judgment
  • Strong communication skills, both oral and in writing, with excellent interpersonal skills
  • Must demonstrate good customer relations skills and a commitment to providing quality service

Description:

A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing. Participates in patient, caregiver, and family counseling and education. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Provides services that are ordered by the physician as indicated in the plan of care. The Skilled professional staff whether direct employee of the agency or under arrangement are expected to contribute to all phases of the QAPI program and participates in with ongoing interdisciplinary assessments of patients. The RN will participate in the development and revision of the plan of care in partnership with the patient, patient representative (if any) and caregiver(s). Supervises care of Licensed Practical Nurse and Home Health Aide.

Qualifications:

  • Currently licensed in the state(s) in which practicing
  • Two (2) years’ experience, hospice or home health preferred
  • CPR certified with American Heart Association or American Red Cross
  • Ability to exercise initiative and independent judgment

 An Occupational Therapist (OT) administers occupational therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing/Clinical Manager. Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s). Participates in patient, caregiver, and family counseling and caregiver education. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. The Skilled professional staff whether direct employee of the agency or under arrangement are expected to contribute to all phases of the QAPI program and participates in with ongoing interdisciplinary assessments of patients. Supervises Occupational Therapy Assistant and Home Health Aide as assigned.

Supervises Physical Therapy Assistants and Home Health Aides as needed. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. The Skilled Professional Staff whether direct employee of the Agency or under arrangement are expected to contribute to all phases of the QAPI program and participates in with ongoing interdisciplinary assessments of patients.

This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s). Participates in patient, caregiver, and family counseling and caregiver education. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Provides services that are ordered by the physician as indicated in the plan of care. The skilled professional staff whether direct employee of the Agency or under arrangement are expected to contribute to all phases of the QAPI program and participated with ongoing interdisciplinary assessments of patients. The LVN will be monitored and evaluated by the Registered Nurse (RN) in the provision of service, provision of treatments, patient education, and communication with the RN, and date collection regarding the patient’s status and health needs as delegated by the RN. Services are furnished in accordance with Agency policies.
